The Role of ChatGPT, Generative Language Models, and Artificial Intelligence in Medical Education: A Conversation With ChatGPT and a Call for Papers
Abstrak
ChatGPT is a generative language model tool launched by OpenAI on November 30, 2022, enabling the public to converse with a machine on a broad range of topics. In January 2023, ChatGPT reached over 100 million users, making it the fastest-growing consumer application to date. This interview with ChatGPT is part 2 of a larger interview with ChatGPT. It provides a snapshot of the current capabilities of ChatGPT and illustrates the vast potential for medical education, research, and practice but also hints at current problems and limitations. In this conversation with Gunther Eysenbach, the founder and publisher of JMIR Publications, ChatGPT generated some ideas on how to use chatbots in medical education. It also illustrated its capabilities to generate a virtual patient simulation and quizzes for medical students; critiqued a simulated doctor-patient communication and attempts to summarize a research article (which turned out to be fabricated); commented on methods to detect machine-generated text to ensure academic integrity; generated a curriculum for health professionals to learn about artificial intelligence (AI); and helped to draft a call for papers for a new theme issue to be launched in JMIR Medical Education on ChatGPT. The conversation also highlighted the importance of proper “prompting.” Although the language generator does make occasional mistakes, it admits these when challenged. The well-known disturbing tendency of large language models to hallucinate became evident when ChatGPT fabricated references. The interview provides a glimpse into the capabilities and limitations of ChatGPT and the future of AI-supported medical education. Due to the impact of this new technology on medical education, JMIR Medical Education is launching a call for papers for a new e-collection and theme issue. The initial draft of the call for papers was entirely machine generated by ChatGPT, but will be edited by the human guest editors of the theme issue.

This paper discusses the use of Artificial Intelligence Chatbot in scientific writing. ChatGPT is a type of chatbot, developed by OpenAI, that uses the Generative Pre-trained Transformer (GPT) language model to understand and respond to natural language inputs. AI chatbot and ChatGPT in particular appear to be useful tools in scientific writing, assisting researchers and scientists in organizing material, generating an initial draft and/or in proofreading. There is no publication in the field of critical care medicine prepared using this approach; however, this will be a possibility in the next future. ChatGPT work should not be used as a replacement for human judgment and the output should always be reviewed by experts before being used in any critical decision-making or application. Moreover, several ethical issues arise about using these tools, such as the risk of plagiarism and inaccuracies, as well as a potential imbalance in its accessibility between high- and low-income countries, if the software becomes paying. For this reason, a consensus on how to regulate the use of chatbots in scientific writing will soon be required.

Abstract On November 30, 2022, OpenAI enabled public access to ChatGPT, a next-generation artificial intelligence with a highly sophisticated ability to write, solve coding issues, and answer questions. This communication draws attention to the prospect that ChatGPT and its successors will become important virtual assistants to patients and health-care providers. In our assessments, ranging from answering basic fact-based questions to responding to complex clinical questions, ChatGPT demonstrated a remarkable ability to formulate interpretable responses, which appeared to minimize the likelihood of alarm compared with Google’s feature snippet. Arguably, the ChatGPT use case presents an urgent need for regulators and health-care professionals to be involved in developing standards for minimum quality and to raise patient awareness of current limitations of emerging artificial intelligence assistants. This commentary aims to raise awareness at the tipping point of a paradigm shift.

ChatGPT is a type of artificial intelligence language model that uses deep learning algorithms to generate human-like responses to text-based prompts. The introduction of the latest ChatGPT version in November of 2022 has caused shockwaves in the industrial and academic communities for its powerful capabilities, plethora of possible applications, and the great possibility for abuse. At the time of writing this work, several other language models (e.g., Google Bard and Meta LLaMA) just came out in an attempt to get a foothold in the vast possible market. These models have the ability to revolutionize the way we interact with computers and have potential applications in many fields, including education, software engineering, healthcare, and marketing. In this paper, we will discuss the possible applications, drawbacks, and research directions using advanced language Chatbots (e.g., ChatGPT) in each of these fields. We first start with a brief introduction and the development timeline of artificial intelligence based language models, then we go through possible applications of such models, after that we discuss the limitations and drawbacks of the current technological state of the art, and finally we point out future possible research directions.

This article has been entirely written by the artificial intelligence (AI) software ChatGPT. This article seeks to describe ways in which AI can be implemented in different fields of healthcare and medical and dental education. The use of AI in publishing is discussed. Importance The rapid expansion of virtual health care has caused a surge in patient messages concomitant with more work and burnout among health care professionals. Artificial intelligence (AI) assistants could potentially aid in creating answers to patient questions by drafting responses that could be reviewed by clinicians. Objective To evaluate the ability of an AI chatbot assistant (ChatGPT), released in November 2022, to provide quality and empathetic responses to patient questions. Design, Setting, and Participants In this cross-sectional study, a public and nonidentifiable database of questions from a public social media forum (Reddit's r/AskDocs) was used to randomly draw 195 exchanges from October 2022 where a verified physician responded to a public question. Chatbot responses were generated by entering the original question into a fresh session (without prior questions having been asked in the session) on December 22 and 23, 2022. The original question along with anonymized and randomly ordered physician and chatbot responses were evaluated in triplicate by a team of licensed health care professionals. Evaluators chose "which response was better" and judged both "the quality of information provided" (very poor, poor, acceptable, good, or very good) and "the empathy or bedside manner provided" (not empathetic, slightly empathetic, moderately empathetic, empathetic, and very empathetic). Mean outcomes were ordered on a 1 to 5 scale and compared between chatbot and physicians. Results Of the 195 questions and responses, evaluators preferred chatbot responses to physician responses in 78.6% (95% CI, 75.0%-81.8%) of the 585 evaluations. Mean (IQR) physician responses were significantly shorter than chatbot responses (52 [17-62] words vs 211 [168-245] words; t = 25.4; P < .001). Chatbot responses were rated of significantly higher quality than physician responses (t = 13.3; P < .001). The proportion of responses rated as good or very good quality (≥ 4), for instance, was higher for chatbot than physicians (chatbot: 78.5%, 95% CI, 72.3%-84.1%; physicians: 22.1%, 95% CI, 16.4%-28.2%;). This amounted to 3.6 times higher prevalence of good or very good quality responses for the chatbot. Chatbot responses were also rated significantly more empathetic than physician responses (t = 18.9; P < .001). The proportion of responses rated empathetic or very empathetic (≥4) was higher for chatbot than for physicians (physicians: 4.6%, 95% CI, 2.1%-7.7%; chatbot: 45.1%, 95% CI, 38.5%-51.8%; physicians: 4.6%, 95% CI, 2.1%-7.7%). This amounted to 9.8 times higher prevalence of empathetic or very empathetic responses for the chatbot. Conclusions In this cross-sectional study, a chatbot generated quality and empathetic responses to patient questions posed in an online forum. Further exploration of this technology is warranted in clinical settings, such as using chatbot to draft responses that physicians could then edit. Randomized trials could assess further if using AI assistants might improve responses, lower clinician burnout, and improve patient outcomes.
